Fertility Florida uses Personal Information to contact you with appointment &/or procedure reminders, important updates or messages, provide you with the Service, process and respond to inquiries, improve the Service, and if necessary.

Children Under Age 13

SMS Terms of Service

Use of the Fertility Florida text message-based services (the “Service”) is subject to the following Terms of Service:

1. Fertility Florida allows users to opt-in to SMS services by sending “TEXT JOIN” to 561-257-0816.
2. To cancel, text “STOP” at any time. A confirmation message will be sent, and you will no longer receive messages.
3. Messages may be delivered to major mobile carriers, including AT&T, Verizon, Sprint, T-Mobile, and others.
4. Message and data rates may apply for SMS messages. Users should contact their wireless provider for details.
5. Fertility Florida is not liable for any costs associated with receiving text messages.
6. You are not required to consent to the Service to receive other services from Fertility Florida.
7. By opting into the Service, you authorize Fertility Florida to contact you via SMS using automated systems.
